Fertivance: The Unani Safoof Tradition and the Science Behind Its Key Herbs

If you have ever visited an old-style dawakhana, you will remember the brass hawan dasta, the rows of glass jars, and the Hakeem sahib telling you to take "ek chamach safoof, neem garam doodh ke saath, subah aur raat". That instruction has hardly changed in hundreds of years.

Fertivance is made in that same tradition: a premium herbal safoof (powder) of 31 ingredients, taken with lukewarm milk. In this article we look at both sides of it. Why Unani medicine prefers this form, and what modern research says about some of its best-known herbs, including the places where the evidence is good and the places where it is still thin.

The short version: Fertivance combines classical men's herbs such as Asgandh Nagori, Tukhm-e-Konch, Musli Safed, Salab Misri, Satawar, Talmakhana and Zafran with nourishing maghaz like akhrot, badam and kaddu. Some of these have encouraging human studies. Asgandh (ashwagandha) has a placebo-controlled trial in men with low sperm count, and walnuts have a randomised diet trial. Others rest mainly on long traditional use. Fertivance is daily nutritional support, best used for several months alongside a healthy lifestyle and proper testing.

Why a Safoof With Milk?

Old knowledge: Unani pharmacy has many dosage forms, such as majoon (herbal paste in honey), habb (pill), sharbat (syrup) and safoof (powder). Safoof keeps the whole herb, not just an extract, so the full plant is taken in. The milk is not an afterthought. In Unani teaching it acts as the badraqa, the vehicle that carries a remedy into the body, and it is itself a muqawwi (strengthening) food that softens the heat of warming herbs. That is why men's formulas are so often taken with milk.
Modern view: Milk adds protein, calcium and some fat, and fat helps the body absorb fat-soluble plant compounds. Powders also allow a larger amount of whole herbs and nuts per dose than a small capsule can hold.

How to Read the Evidence

Label What it means
Human trial Tested in men against a placebo or control group
Early human study Studied in men, but without a placebo group or in very small numbers
Traditional use Long use in Unani practice, with little modern testing so far

The Key Herbs, One by One

Asgandh Nagori (ashwagandha) Human trial

Old knowledge: Asgandh entered Unani practice in the subcontinent from the older Indian tradition, and Hakeems have long used it for strength, nerves and weakness in men. The name Nagori refers to the prized variety from Nagaur in Rajasthan.
Modern research: In a randomised, double-blind, placebo-controlled pilot study, 46 men with low sperm count took 675 mg a day of ashwagandha root extract for 90 days. In the ashwagandha group, average sperm count rose from about 9.6 to 25.6 million per mL, semen volume from 1.7 to 2.8 mL, and motility from about 19% to 29%. Testosterone also rose modestly. (Ambiye et al., Evidence-Based Complementary and Alternative Medicine, 2013)

What this means: it is one of the most promising herbal findings in male fertility, though it was a small pilot study using a standardised extract, so larger trials are still needed. Read more in Benefits of ashwagandha.

Tukhm-e-Konch (Mucuna pruriens) Early human study

Old knowledge: Konch seeds are a classic ingredient in subcontinental men's tonics, traditionally used for vitality and desire.
Modern research: Researchers in Lucknow gave infertile men Mucuna seed powder (5 g a day for three months) and reported lower markers of stress along with better sperm count and motility. The study compared men before and after, and against healthy fertile men, but did not use a placebo. (Shukla et al., Evidence-Based Complementary and Alternative Medicine, 2010)

What this means: interesting results, especially the link between stress and semen quality, but not yet proof.

Akhrot (walnut) and other maghaz Human trial

Old knowledge: Fertivance includes a group of nourishing seeds and nuts (maghaz): akhrot, badam, pista, findak, kaddu and others. Unani physicians valued maghaz as rich, strengthening foods for the brain and for male vitality.
Modern research: At UCLA, 117 healthy young men eating a typical Western diet were randomised to add 75 g of walnuts a day or to avoid tree nuts for 12 weeks. The walnut group showed better sperm vitality, motility and shape, while the control group did not change. The researchers note it is not yet known whether the same benefit applies to men with fertility problems. (Robbins et al., Biology of Reproduction, 2012)

Pumpkin seeds (kaddu) are also one of the richest plant sources of zinc, a mineral the body needs to make sperm.

Zafran (saffron) Mixed results

Old knowledge: One of the most precious Unani ingredients, used as a mufarreh (exhilarant) to lift the heart and mood, and in small amounts in many men's formulas.
Modern research: Saffron is well studied for mood. For semen quality the results have been mixed: an early small study reported better sperm motility and shape (Heidary et al., 2008), but later controlled research has not consistently confirmed this. We include Zafran for its traditional role, not as a proven sperm booster.

Musli Safed, Musli Siyah, Salab Misri, Salab Panja, Satawar and Talmakhana Traditional use

Old knowledge: These roots and seeds form the backbone of classical Indo-Unani men's tonics. Musli and Salab are prized restoratives, Satawar is a cooling, nourishing root, and Talmakhana seeds have long been used for male strength. Some have been studied in laboratory and animal research, but human trials are still limited.

Javatri, Mastagi Roomi and the balancing herbs Traditional use

Old knowledge: Warming spices like Javatri are paired with digestive herbs like Mastagi Roomi and cooling ingredients like Satawar and Tukhm-e-Sars. This follows the classical method of compound formulas (murakkabat) described by Ibn Sina in Al-Qanun fi'l-Tibb: active ingredients, helpers, and correctives working together so the formula suits long-term use.

The full ingredient list, along with the product's registration details (NCT No: QH-52872 A, PHC No: R 96547), is on the Fertivance product page.
