PCOS in Pakistani Women: Symptoms, Causes and Natural Management Options

In short: Polycystic ovary syndrome (PCOS) is a common hormonal condition that can cause irregular periods, excess hair, acne, weight gain and difficulty conceiving. South Asian women have a higher tendency toward insulin resistance, which is closely linked with PCOS. It cannot be cured, but symptoms can be well managed with lifestyle changes and medical care.

Common Symptoms

  • Irregular, infrequent or absent periods
  • Excess facial or body hair (hirsutism)
  • Acne and oily skin
  • Weight gain, especially around the waist
  • Thinning scalp hair
  • Dark patches of skin on the neck or underarms (a sign of insulin resistance)
  • Difficulty getting pregnant

What Causes PCOS?

The exact cause is not known. Key factors are:

  • Insulin resistance: high insulin pushes the ovaries to make more androgens (male-type hormones)
  • Higher androgen levels: disturb ovulation and cause hair and skin symptoms
  • Genetics: PCOS often runs in families
  • Lifestyle: inactivity and weight gain can worsen symptoms

How It Is Diagnosed

Doctors commonly use the Rotterdam criteria. PCOS is diagnosed when at least two of these three are present, after other causes are ruled out:

Criterion How it is checked
Irregular or absent ovulation Period history
High androgens Symptoms or blood tests
Polycystic-appearing ovaries Ultrasound

Natural Management Options

  • Losing 5 to 10 percent of body weight can help restore cycles
  • Low-sugar, high-fibre diet: daal, sabzi, whole grains, less maida and mithai
  • 150 minutes of activity weekly
  • Good sleep and stress control

Unani perspective: Unani medicine describes conditions with irregular or scanty menses (qillat-e-tams, ihtibas-e-tams) and links them with imbalance of temperament and accumulation of thick humours. The Unani approach focuses on regimen, light diet, exercise and herbs to restore balance, similar to modern lifestyle-first PCOS care.
